Centre Aid Form

Our institutional form used to report additional financial and family information not requested on the FAFSA. Every student applying for need-based aid is required to complete this form each year (2 pages). This form should be returned to Centre’s Student Financial Planning Office when you have completed it.

Verification Worksheet

One of every three students who files the FAFSA is selected for review in a process called verification. A comment will appear on the front page of your Student Aid Report (SAR) if you have been selected for verification. Aid applicants who are selected for verification must provide additional documentation to Centre before they can receive a financial aid award. Specifically, students selected for verification must either use the IRS Data Retrieval to complete their FAFSA OR mail us their IRS Tax Return Transcript. Click here for instructions. The Verification Worksheet must be completed and signed by the student and at least one parent (2 pages). This form should be returned to Centre's Student Financial Planning Office when you have completed it.
