Submit Health Records
All first-year and transfer students are required to submit current health records prior to attending Centre.
Forms and Questionnaires
Complete or upload the following information through the Student Health Patient Portal.
- Medical History Form and Medical Insurance Information:
Answer questions and upload a copy of the front and back of your insurance card. Required to keep health insurance information updated. - Tuberculosis Screening Questionnaire: Answer questions and provide additional information if requested.
- Immunization History Form: Add dates and upload a copy of your immunization record.

Required Vaccines
Meningococcal (ACWY)
One dose on or after 16th birthday, examples Menactra®, Menveo®, and MenQuadfi®.
Meningococcal Vaccine Information Statement
Measles, Mumps, and Rubella
You must complete at least one of the follow three options to satisfy this requirement:
- Two Measles-Mumps-Rubella (MMR) MMR vaccines Vaccine Information Statement OR
- Titers (lab results) showing immunity to measles, mumps, and rubella OR
- Two doses of Measles vaccine & two doses of Mumps vaccine at least 28 days apart after 12 months of age & one dose of Rubella vaccine after 12 months of age.
Tetanus
Within the last 10 years (Pertussis preferred, i.e. Tdap, examples Adacel and Boostrix)

Strongly Recommended Vaccines
- Up to date COVID-19 ('23-'24) Bivalent Vaccine
- Hepatitis A
- Hepatitis B
- Human Papilloma Virus (HPV)
- Flu shot (when the '24-'25 is available)
- Meningitis B (if not included in meningitis vaccine with ACWY)
- Polio
- Chickenpox (Varicella)
