Forms and Questionnaires

Complete or upload the following information through the Student Health Patient Portal.

• Medical History Form and Medical Insurance Information:
  Answer questions and upload a copy of the front and back of your insurance card. Required to keep health insurance information updated.
• Tuberculosis Screening Questionnaire: Answer questions and provide additional information if requested.
• Immunization History Form: Add dates and upload a copy of your immunization record.

Required Vaccines

Meningococcal (ACWY)

One dose on or after 16th birthday, examples Menactra®, Menveo®, and MenQuadfi®.
Meningococcal Vaccine Information Statement 

Two newer vaccines offer protection against meningococcal serogroups ABCWY, Penbraya® and Penmenvy®. These will also satisfy this requirement.

Measles, Mumps, and Rubella

You must complete at least one of the following three options to satisfy this requirement:

1. Two Measles-Mumps-Rubella (MMR) MMR vaccines Vaccine Information Statement
   OR
2. Titers (lab results) showing immunity to measles, mumps, and rubella
   OR
3. Two doses of Measles vaccine & two doses of Mumps vaccine at least 28 days apart after 12 months of age & one dose of Rubella vaccine after 12 months of age. 

Tetanus

Within the last 10 years (Pertussis preferred, i.e. Tdap, examples Adacel and Boostrix)

• Annual COVID-19 Vaccine
• Hepatitis A
• Hepatitis B
• Human Papilloma Virus (HPV)
• Annual Influenza/Flu Shot
• Meningitis B (if not included in meningitis vaccine with ACWY)
• Polio
• Chickenpox (Varicella)
